Course Registration
Complete the form below for course registration. Use the slider at the right of the form to scroll to the bottom of it.
In order to complete your enrollment/registration, SelfDesign High must have (a) a copy of your birth certificate on file and (b) a completed learning plan of all your past and current high school courses and credits. These items can be provided either by email to admissions@selfdesignhigh.org or faxed to our office at 1-866-910-8869. Please note that new registrations will not be processed until these items are on file.
To complete your learning plan:
- Download the Learning Plan template *
- Fill in the required fields
- Save your form to your desktop
- Attach to an email and return to admissions@selfdesignhigh.org
